How does my HSA activity get reported for tax purposes?
The trustee/custodian of the account provides you and the IRS with IRS Form 5498-SA reporting all contributions to the account, as well as IRS Form 1099-SA reporting distributions from the account. Tax-free contributions are reported on your W-2. Use these documents to complete IRS Form 8889, which is submitted with your Federal tax return. IRS form 8889 must be filed if any of the following apply:
- You (or someone on your behalf, including your employer) made contributions to your HSA.
- You received HSA distributions.
- You must include certain amounts in income because you failed to be an eligible individual during the testing period.
- You acquired interest in an HSA because of the death of the account beneficiary

You will be receiving information from your employer regarding your previous year’s tax information. In addition, you will also receive all pertinent information regarding your HSA. Follow the directions, or have that information available for your tax preparer.
